Where are we located?

What is the phone and fax number?

What are your office hours?
8:00 AM to 4:00PM Monday through Friday. However, due to surgery times throughout the week not all providers are available at all times. Please contact the office for additional information.
Do I need a referral?
Many insurances require a referral for specialist services. If you are unsure if your insurance requires a referral contact your insurance provider. Then obtain a referral from your primary care provider and have it faxed to our office at 272-1260.
What insurances are accepted?
Adults: MVP, CDPHP, BSNENY, Medicare, GHI, GHI HMO (not the government run programs), Empire Blue Cross Blue Shield, NYS Empire
Children: Dr. Slavin will care for children with orthopedic concerns regardless of insurance coverage. Please contact our office for more information.
What should I bring with me to my appointment?
- Insurance cards
- New patient form (get form here)
- Insurance form (get form here)
- X-rays, MRIs, other tests and results
- Lists of medications
- Copayment- accepted forms of payment include: cash, check, and credit or debit cards in form of Discover, Mastercard, and VISA
What if I am unable to make a scheduled appointment?
Please call the office 24 hours prior to your appointment as a courtesy to staff members and other patients who may be placed in that time slot.
What services are offered?
A wide range of orthopedic, pediatric orthopedic, and sports medicine services including but not limited to the following: arthritis care, fracture care, sprains, strains, treatment of upper and lower extremity joint problems, cerebral palsy, children with myelomeningocele, developmental hip dysplasia, pediatric fractures, sports medicine, complex pediatric orthopedic problems, recreational, high school, and professional athletes with injuries, the Ponseti method for club foot treatment, and total joint replacements including the use of the Otis med knee system.
Do you do x-rays at your office?
Yes our office is equipped with a radiology suite for our patients’ convenience and patients requiring x-rays who have not already had them can have them at the time of their visit.
